Some that I'd add.
12: Shinji - he failed an identical Warped Wall to one he cleared in 11, so he likely just got unlucky by not getting it in his first few attempts and then gassed out. Hard to say though as he was All-Cut. 18: Bunpei Shiratori - him suddenly slipping off the spider walk seemed like a freak accident as he'd never had issues with it before. 22: David Campbell - he just spent too much time on the Slider Drop getting caught in the net but was otherwise on track to clear. 27: Katsumi Yamada - had a couple of close attempts on the wall and because of the lenient time limit would've had enough time left to clear. 28: Mohammed Farid Isham - he spent 20 seconds after the Escargot wincing in pain, had this not happened he would've had enough time to clear. 33: Kawaguchi - made a completely avoidable rookie error by just going for the Double Pendulum jump without a backswing. Would've cleared otherwise as he did in the other 6/7 tournaments from 30-36. 37: Ashlin Herbert - he wasn't aware of the timer for some odd reason so just took way too long between obstacles, he would've cleared otherwise as he got the wall in one shot 37: Ali Hay - was on track for the fastest time but had an ankle injury that screwed him for the wall. 37: Iwamoto - he very nearly cleared the Dragon Glider but just got his back foot stuck in the water. 39: Araki - yes, he had to run in the rain, but after he timed out he actually cleared the wall, so was clearly affected by the time pressure. It had also just started raining then so the wall was mostly dry with wet patches, unlike Yusuke's run where the wall was actually soaked/slick. 39: Wasabi - same as Iwamoto in 37. 40: Iwamoto, Sugeta, and maybe Horishima - grouping these together as they all got screwed by the Dragon Glider derailing. 40: Hitoshi Kanno - his first attempt on the wall was agonisingly close 40: Yusuke Goto - accidentally pushed the bar away instead of grabbing it rendering him unable to grab with his other hand; seemed like a freak accident/choke.
Would also add Kota Sakuma for all three of his wall fails.
That said, it depends how you define unlucky though in this case. I was on the side of caution in the sense that the only ones I put down were competitors who would've very likely cleared had it not been for an unlikely external factor or REALLY bad luck like just missing the wall.

I think Kawahara Takuya at least have 2 tournaments that he can clear Stage 1.
