What if Yamada cleared the Jump Hang in Sasuke 14? Would it have been his last clear instead of Sasuke 12? I can think of three possibilities: Warped Wall time out or Stage Two time out or Stage 3 fail(after the Cliffhanger)

I think yes. He is still confident at that time, and given he actually reach Jump Hang rather quick, he had enough time to clear. You see that he approach Jump Hang more slowly when he met Jump Hang again, that approach backfire him at SASUKE 16.

It’s honestly hard to say because even when he did clear the Jump Hang/Jumping Spider he would time out by one means or another. I always feel like after Sasuke 14, Yamada either went for either technique or speed in his runs, but couldn’t get both. When he was fast, he’d often fail at the Jumping Spider or Warped Wall. When he did obstacles right, he often went way too slowly.
I do think though that his subsequent failures were a function of both his age and decreasing confidence from a larger string of past Stage 1 failures. Also I think his retirement during 13 had an effect, given that he didn’t seem to have any problem with Stage 1 EVEN during the 13 trials.
