When I was laying in my bed,I watched Asaoka Hiroyuki fail the Jump Hang in SASUKE 6.Then I thought"Wow,that was so dumb".Then,I thought of creating a new thread.

It's gotta be Tajima, literally choked at an almost guaranteed Final Stage attempt because he forgot to unhook a strap. Or Kong in Final Stage in 24 when he tried to climb his safety rope

He just slipped at the top of the Rope Climb, when the course was wet, no less. I don't think 31 was that bad either, considering Takeda did exactly the same thing, IMMEDIATELY after. I agree with your other four choices though. Anyway, to add a few of my own, Katsumi Yamada in 9, Bunpei Shiratori in 21, James Okada in 7, Toyohisa Ijima in 11 (sort of...), Katsuhide Torisawa in 27, Yasuo Aoki in 24, Yanagama Masaharu in 7 (the guy who pulled the rolling log the wrong way), Shoei in 6, that guy in 17 who made the jump onto the Circle Slider then forgot to let go, that Chinese acrobat in 2 who leapt face first into the Barrel Climb, that comedienne in 18 who let go of the Rope Glider almost immediately, and the stuntwoman (I think) who didn't let go at all, and Shingo Yamamoto (yes, him again) in 21. And I might give an honorable mention to Tomohiro Kawaguchi in 21. (I can also think of at least one from Sasuke 33, but alas, that would be a spoiler...) There's plenty more, but I spent long enough thinking of those ones.

Kong's Final Stage botch wasn't on him, it was on the crew worker who let the two get tangled. He would have arguably come very close to winning based on his pace.
I think Ishikawa in 27 is an oft-missed one. He didn't just swing off the notch without jumping, he swung backwards and flipped with the bar in his hands.

Ishikawa's fail wasn't "dumb". He just couldn't do it, probably due to a mental barrier. It's a pretty intimidating obstacle.

Not Sasuke but worth throwing in due to related: my #1 all time fail is the dude in VIKING who held the wrong end of the pegs on the peg board/monkey pegs and fell immediately into the water
