In a sense that alot of obstacles are kept that I'd like to see replaced, yes, it is the same every year. But that's only about 30% of the course.

I haven't seen ANW6's course yet, but from ANW4 to ANW5 it was very minimal change compared to what you see in Sasuke. They only changed the first 3 obstacles in stage 1 and the first of stage 2. Over 80% of the course (20 of 24 obstacles) were technically the same, but had tweaks in design that affected difficulty such as the balance tank's slope and the salmon ladder rung's material and angle.

The first three obstacles in the first stage are different stage two and three also have different obstacles they were shown in the clip show last week.

I think s1 ob2 is the same but 1st and 3rd are changed in s1, 1st and 4th in stage 2, and not sure how much changed in stage 3 but they showed new obstacles, I think 1st, 2nd and 5th obstacles are new
