Well, there's the 3 in 18 (Nagasaki, Takahashi, Nagano), none in 19, technically two in 20 (Levi and Okuyama), and 4 (Lee, Miyazaki, Takeda, Nagano) in 21. So 9.
Sasuke 3 - Stage 4.
How many times did Takamasa Nagasaki mess up on the Warped Wall before clearing it and failing the Flying Chute in Sasuke 19?

Well that can really have two answers. One is Kazuhiko Akiyama in Sasuke 9 when he failed the Quintuple Step. The other is also Kazuhiko Akiyama but in Sasuke 20. Since he went first that day, he technically went the "earliest" ever since an All-Star usually doesn't have that low of a number. He failed the Half-Pipe Attack that day.

Guess I worded that bad, I meant whats the earliest obstacle an All Star failed on, so you still are correct on the first one.

My turn to ask the question: What was Hiroyuki Asaoka's last tournament and where did he fail? I think it was in 15 and he failed the First Stage's Rope Climb. Am I right? If so, Sasuke 2 - Stage 1. And if I am right, my ? is this: Which two All-Stars did not clear the First Stage on their very first try at Sasuke?
