Was he allowed a second chance in the Stage 2 or not (because of the Stick Slider defect)? Or did he just not bother if he was allowed one? (Or was it cut from broadcast?)

It was not cut from the broadcast. That was his only run in Sasuke 20 (remember that G4 always cuts a lot so go back to the TBS broadcast to see what happened). Thanks to that though they changed the stick slider so the bar has stoppers at the end and not like the old pipe slider set up where there are no stoppers. There was no "defect" per se other than the night was a bit misty and any metal object had issues with getting stuck. That's the price you pay for having an outdoor course.

Ahh ok, thanks. So it was one of those defects that you had to work with because everyone would face the same problem. Thus, no second chance unlike Nagano's Slider Jump defect in 23 which made it impossible to clear and was considered an unusual condition.

Similar to the Pipe slider where people came off the rails if they were not perfectly straight this is what happened. Since then they changed the design to also fit the Salmon ladder. It's always a work in progress and it's painful when you see things like this happen. I heard they had issues with it even before they took to it but since it was due to moisture from the night air.. and not necessarily a defect (like Nagano's cord being wrapped up behind the screw thus shortening the length of the safety rope) they let it stand. Actually this is also the same problem that the Slider Drop had with Levi. He expected it was going to stay straight by virtue of the rails alone and did nothing to compensate for the drop. That was human error rather than obstacle error.

Originally it was suppose to be part of the obstacle, you can tell that from the TBS broadcast of Sasuke 18 because they show a tester failing it the same way, so obviously they intended people to do that. Apparently Okuyama's fail made them rethink this plan.
