Design a theoretical renewal course using only real Sasuke obstacles that would make H&S and competitors alike s*** their pants. Course should be dangerous and scary, something that should never see the light of day, but also borderline realistic.

I'm glad you mentioned that the course should be borderline realistic. As Igor Stravinsky stated once: "The more constraints one imposes, the more one frees one's self. And the arbitrariness of the constraint serves only to obtain precision of execution." - bit of a tangent but basically it's more interesting when you can't just go the default route of making everything as hard as Sasuke 33's Stage 3.
Yours is pretty stacked so I've got my work cut out. Here's mine.

With Stage 1 I basically just crammed in all of the most brutal obstacles in history into a single stage, with most of them unsurprisingly derived from Sasuke 19. Starting with the Rolling Escargot might be a bit controversial but one of my main complaints of the current Stage 1 is that the first half is too easy and results in too many drawn out runs c/o joke competitors and rookies, whereas putting such a hard obstacle first gets rid of the fodder ASAP which I feel would be refreshing. The Jumping Spider and HPA are two of my favorites that I miss so I added the hardest version of those in there as well. I also feel like the Dragon Glider wouldn't be the major focal point of Stage 1 being surrounded by other equally or more hard obstacles so it wouldn't just be a case of 90% of competitors failing there.
For Stage 2 I wanted to go back to basics. No Backstream or anything; just a good old speed stage, starting with the original long a** Spider Walk such that unless you get through it absolutely rapidly you're going to have a hard time getting through the rest of the stage. I also added some more failable obstacles to the back half of the stage to penalise careless rushing in a stage where you have to rush, which is what I always loved about the older versions of this stage.
For Stage 3 I kept the idea of joining the first two obstacles, but this time went with the Floating Boards as I felt as though they were criminally under-utilised in the show. I also always loved the idea of joining the Cliffhanger and Vertical Limit, but felt that the 32- version with the two gaps was way too hard; I think one of the earlier variants is still brutal as f*** but more realistic and wouldn't result in a massive stagnation in results. I also put the Swing Edge as the final obstacle after the Flying Bar, because it would be brilliantly cruel to end the stage on such a nail-biting obstacle especially as your grip will be absolutely fried after the rest of the stage and having to do reverse laches on ledges would just be impossible.
For Stage 4 I just kind of spitballed. Only real change is the Lumberjack Climb which I would've always liked to see as an alternative Final Stage obstacle. I feel like the rock wall is a bit unfair as it basically locks anyone without a climbing background out of Kanzen. Sasuke's always been about welcoming multiple disciplines and a test of athleticism and dedication more than anything, not whether you go bouldering every weekend or not.

Stage 1 was directly modeled after SASUKE 19, with some exceptions. For the most part, I chose the Rope Glider, mainly down to how dangerous it was in SASUKE 18(Five competitors were injured mind you). Both the Pole Maze, Jumping Spider, and Flying Chute were taken from SASUKE 19. I decided to put the Iron Paddler in Stage 1, mainly down to strain on the triceps and also put it directly before the Double Warped Wall. And lastly, I put Free Climbing as the last obstacle, because it is an criminally underrated obstacle from the first 2 SASUKEs. The time limit would be 90 seconds long for Stage 1.
For Stage 2, I wanted to crank up the difficulty even more. The Spider Walk is the one used from SASUKE 1 to 4, the long version. I went with the Prototype of the Salmon Ladder, mainly down to the rungs being equal in length, and the massive gap from the seventh rung to the Stick Slider. And then, I put the Tackle directly behind the Wall Lift, and the version is from SASUKE 5, where it is a 50KG box and the Wall Lift is as classic as dust: No need to replace it. Time Limit would be 70 seconds long for Stage 2.
Stage 3 is a strength drainer and I wanted to epitomize it. After the Ultimate Cliff Hanger, there is no more breaks. I know some are saying, "Has Furatachi gone mad with the Hang Move?" LET me explain my stance. The SASUKE 2 to 3 version had the competitors grab onto the footholds with their feet. In my version of it, directly after the UCH, competitors can only use their hands on the Hang Move, as a grip strainer. The Sending Climber should have stuck around for longer, as it would be a much harder Hang Climbing. Vertical Limit directly after it and then the old classic in the Pipe Slider, to completely tax your grip even more.
The Final Stage is only 50 seconds long, but 32.5 meters in height. The Spider Climb from SASUKE 5 to 17 is kept, as well as the 50 cm gap, the Salmon Ladder Jugo Dan is 10 meters long, and then an 10 meter Pole to climb up for the buzzer. I have always been fascinated with KUNOICHI and the pole they had for the last obstacle in Stage 1 as well as in KUNOICHI 6 would be much harder for the men.

This should be fun.
1st Stage - 100 to 105secs

Starting off with 2 of the shows more unique obstacles, a serious h&s hazard followed by a potential h&s hazard and if nothing else, your forced to run as fast as possible to clear the jump, so it's an endurance test straight away, easier if your not winded or injured from the Rope Glider. Followed that up w/ the sk26 Rolling Escargot and then the SK19 Jumping Spider, 2 incredible shin era obstacles from Shin II and Shin I respectively. I specifically chose the Halfpipe Attack from SK27 because of the landing platform being solid wood. Perfect landing material for glass knees, as demonstrated by that bastard. The Tackle to Wall combo has aged beautifully well, an extra wall means an extra stamina test for those who don't have the greatest lower body. Then to add some more fun, the Flying Chute from SK19, normally I wouldn't go beyond 8 obstacles for any stage, but considering what the purpose of these blueprints are, it would be very wrong not to have it here. Finally to cap it all off, the best final obstacle the first stage has had, the Lumberjack Climb. If Injuries weren't a problem, I feel this would be a good stamina and endurance stage, with injuries, this has a shoulder breaker, a nagano injury maker, a yamada ankle sprainer, a bastard knee shredder, and some obstacles that would be a nightmare drenched in rain. The Rolling Escargot is there too I guess.
2nd Stage - 90 to 95 secs
1 - Slider Drop 2 - DSL + 3 - Unstable Bridge 4 - Backstream (SK29 Version) 5 - Balance Tank (SK 10-17 Version) 6 - Metal Spin (SK 18-27 Version) 7 - Wall Lift (pre nerf)
So to start, the slider drop and it's exit drop has caused an injury, although considering how awkward the landings where and seeing guys like Okuyama take that fall and still be fine, albeit a tad winded, I'm surprised it didn't cause more injuries. In general the layout for the first 3 obstacles is the same as the layout done during Shin II. But then we have the remaining four, the Backstream in SK29 was insane, Koji Hashimoto's reaction to Yuuji's Backstream attempt pretty much sums up that version of the obstacle really well. Because of the stipulation that h&s can go screw themselves, it is then immediately followed by the original Balance Tank which means slippery feet and an elongated, rope less Balance Tank that should in theory mean more Yamada and Shiratori like moments. After that is the Shin Era metal spin, a nice grip test considering the wet hands and finally the pre nerfed Wall Lift, meaning that the Walls don't have a delay when they drop and thus it is more visually appealing, plus as Levi Meeuwenberg has demonstrated, can cause injury.
3rd Stage
1 - Arm Rings + 2 - Roulette Cylinder + 3 - Sidewinder (SK 39 Version) 4 - Cliffhanger Dimension (preferably with the slippery ledges from SK38) 5 - Vertical Limit Kai 6 - Spider Flip 7 - Swing Edge/Flying Bar/Pipe Slider (with SK17 Dismount)
Before I break the course down, I was really tempted to add in the SK29 time stipulation. Just Imagine, You go through a course designed by people who don't give a damn but you conquered the 3rd stage which means your dream of attempting the Final is within your grasp, oh wait, Yusuke cleared the third stage in a faster time...Sry, better luck next time. But in fairness, despite how beautifully cruel that is given the stipulation, it's just not right at all, it was stupid then, it might not be stupid now given the task at hand, but for the sake of the theoretical competitors, I'll let it slide.
Anyways, my third stage. First up the Arm Rings which means a nightmare for people with either wrist or shoulder problems, followed by the SK26 Roulette Cylinder and then directly into the most recent incarnation of the Sidewinder. As you may have noticed that's 3 obstacles without a resting area, but as done by a lot of people who have attempted the Sidewinder in any of it's incarnations, it is possible to rest on the poles. After a much needed rest comes the Cliffhanger Dimension (with the ledges being slippery like in SK38) then into the Vertical Limit Kai, and then the Spider Flip, which means a scary fall if someone fails the transition. Finally, the above mentioned trio of final obstacles, I'm fine with either one as long as they all have the SK17 dismount.
4th Stage - 35 to 40secs
1 - Spider Climb (If possible with splitting walls) 2 - Lumberjack Climb 3 - Salmon Ladder Jugo Dan 4 - Rope Climb (Particularly the Rope Climb from the original Shin Final Stage, the one shown during SK17's closing credits)
I don't really know how tall this stage could possibly be, so I didn't bother with heights and stuff. So the first element of this tower is the Spider Climb and in particular, preferably the one with original panels which means that after a certain amount of time it split, but that would mean a tall enough Spider Climb to make it even worth the effort for the split. Really the first 3 (and really the entire tower) is pretty much the same as Dakoosu's, however for the rope climb, I preferably want rope's to be in a similar layout to that of the original Shin Final Stage, aka the one shown during the closing creds of SK17.
For the most part I designed each stage with a combo of being realistically hard while also being physically demanding, and hopefully I tried to emphasize the lack of care in terms of H&S enough, despite how circumstantial the injury potential is. All in all, I think it's reasonable enough, not too crazy but still clearable... I hope.
