What would happen if a competitor made it to the top of the stage 4 tower, got unhooked and either lost balance or concentration and fell through the circular ring where the rope is lowered?

Well....
The show would get cancelled for sure, even if it was entirely the competitor’s fault.
Also, not that they would air that competition but if they did they’d act as though the competitor never competed.

I always get a chuckle seeing the helmet on the reporter's head in Stage 4. As if that'll save them if they fall.

The issue is Salmon Ladder Jūgo Dan. That would be a ninja killer (in a literal sense).

In fairness... there is a giant crash mat at the bottom of the tower, or at least there used to be. That could potentially be enough to break a fall?

Depends on how you land. Best way to survive a big fall is to land feet first, knees instinctually moderately bent, and falling on your side after a likely bounce off the ground while covering your head. Also slamming into the Salmon Ladder would actually help break your fall up and potentially raise your chance of survival, especially if you tried grabbing it. That's what the internet tells me anyway.
Some people die just falling down half a flight of stairs.
