I think there have been only three: Shinji Kobayashi (11, 14), Bunpei Shiratori (12, 13, 15, 16) and Kazuhiko Akiyama (4, 12). None of them have ever fallen in on the Cliff Hanger.
One could argue that Nagano hasn't failed it either, but technically he has: in 18, when he grabbed the top of the ledge and disqualified himself, that would count as a fail IMO. So for my answer, it's the three mentioned above.

The Brick Climb in Stage 2 and the Log Pole thing that was in the first two competitions. Those aren't obstacles at all. Hence the reason I chose them. The question (best I can understand) is he's asking for obstacles that could be attempted without actually being attempted. The log pole could be skipped entirely (but was actually considered an obstacle in the first Sasuke) while the Brick Climb was always used but not an official obstacle. Everyone's taking their stabs in the dark at this, that was mine. It isn't my fault if everyone is so desperate to come up with hard questions that they'll ignore every grammatical and logic concept possible in sentence and question structure. How many of the last forty or so posts is people asking for clarification on a question because the asker couldn't think up a simple question with a tough answer and instead look for a simple answer with an impossible to understand question?

Do you mean ones that were done but got through so easily and no one ever failed them? Like the Needle Climb (Eye of the Needle according to G4), The Chain Swing, the Tackle Machine. the Ledge Walk, The Arm Bike, The Bungee Bridge, The Net Bridge, The Shoulder Walk, the Curtain Swing, the Swing Ladder, the Downwards Globe Grasp and the Spider Flip? EDIT: BTW, Learn some damn english man!
