I'm not sure if this question was ever answered, but there were a few instances where the Rolling Maruta would come off its track and cause competitors to fall into the water without being able to do anything about it, such as Ken Yasuda in Sasuke 5:
Or especially Kenjiro Ishimaru in Sasuke 13 where he failed the Sandan Rolling Maruta where the drops caused his fall.
What I want to know is, were any of these people who fell into the water given a second chance since they technically didn't fail an obstacle since the obstacle was faulty?

That answer is simple. No they didn't get another chance. The reasoning behind it was that they incorrectly did the obstacle causing the obstacle to shift on it's own because it was either mounted incorrectly or because they were falling off their body weight shifted. If they mounted it correctly (between the white lines (or were they yellow?) that are visible when you get on it) AND are strong enough to keep straight then it won't come off. Any variation of weight causing it to dislodge was because they were falling off it.
Painful though.. I think that's one of the reasons why it was replaced. Given Ishimaru's almost disastrous exit from it I'm sure the designers were thinking "okay .. this has got to go".

Well it lasted for 5 tournaments though, from 13 to 17 so I'm surprised they didn't take it out sooner from what you are saying by that comment.
However, I don't recall seeing the log coming off its track after the 13th tournament from 14 to 17, so they must've done something to fix it.
BTW, is it just me or does it seem like the log itself looked like it weighed a lot less in the earlier tournaments?

Just from looking at it from old videos that sucker has gone through a lot of changes. Everything from being lighter, made of different materials to just the track being very different each time they did it. You can tell they tried to improve it and each time caused a different problem than before. I'm sure once Nagano won they wanted to get rid of those obstacles and try something completely new.
