His Sasuke 20 defeat kind of knocked him down a few pegs in confidence that is why he did not complete the first stage in 21. We can't really be sure of that. I just noticed Takuya Kawahara (#60) on the list. I can't believe he's never passed the first stage. And that course tester on the Cliff Hanger looked so much like him as well, lol. Yuta Adachi (#69) as well! His third time. Was he in the broadcast of 18? I've only seen highlights of that tournament. And one thing, the list has Brian Orosco down as a first timer.

So anyone have a clue on where Okuyama failed? And what is this talk about Washimi?

In the G4 broadcast I saw an aerial shot of someone who looked very much like Okuyama failing the Jumping Spider. There wasn't a clear shot of the number.
Then again, I also saw someone who looked like Takuya Kawahara failing the Flying Chute but it turned out to be someone else.

If you watch Dante's run on Sasuke Maniac, you can see someone passing the Jumping Spider on the big screen behind Dante. This is Yuji Washimi, now where he actually failed is a mystery but we can conclude that he made it to the Halfpipe Attack.
Also, I'm confident that whoever that is, it is not Okuyama. For one, the sun is still high in the sky and most of the later people competed in the shade. Plus, I remember seeing this person in the Japanese broadcast.

The monitor didn't show up well on camera (visible, but barely), and I don't recall it showing until the end of the jumping spider, you see someone on it but not passing, its possible to fail outside of the jump, There was someone in 20 who failed like that if I remember correctly

LOL Bada that's funny But yeah I had that reaction when seeing the list originally. Half the time I was staring at the column of times on Sasuke going "wow he got cut more times than I thought! He must have pissed of TBS big time! LMAO" (meaning Shinji).. Still cracks me up he had the balls to show up to Sasuke with a "Sasuke Baka" T-shirt. I want that T-Shirt

I think he means perfect attendance since his debut in Sasuke 11. However, he has missed one whole tournament. I think it is believed to be Sasuke 17.

Unfortunately no. Unless we know of someone who was there AND paying attention specifically to them we don't know for sure.
